At-Need Arrangements
After a death has occured, the first step is to contact your funeral home of choice.

The following is the needed information that we will use to prepare the obituary and the death certificate.
Once you have completed the Worksheet, please fill out the remaining information provided on the list below. You may either email this information to your funeral home of choice, or bring it to your appointment.
- Please complete this Worksheet. We will use this information for the death certificate so please be as thorough as possible
- List of Survivors (If applicable)
- Spouse
- Children
- Grandchildren
- Siblings
- Great-Grandchildren
- Any others such as nieces, nephews, or significant others
- List any church or fraternal connections
- Special hobbies or remarks to be added in the obituary
- Place for memorials to be sent in memory of the deceased
- If services will be a traditional burial, please look at our selection of caskets.
